NEW PATIENT REGISTRATION
Your first visit is an important one and helps to establish the foundation for your relationship with your care team. Following is helpful information such as new patient forms, a map and directions to the office of your choice, practice hours, payment policy and more.
1) Choose a LRMC location
We have several locations throughout Horry County to serve you and your family. Please choose the location that provides the service you need and is most convenient to you. To see each location, click HERE.
2) Check with your insurance company
If you are insured, check with your insurance company to see if Little River Medical Center is considered in-network with your plan. This could have an impact on your out-of-pocket expense.If you do not have insurance, we will be happy to assist you with applying for our sliding fee program or other insurance options. If you need assistance with the sliding fee application, we suggest you meet with a member of our outreach team prior to your appointment.

3) Request an appointment
Once you have selected a location, you can request an appointment one of two ways. You can make a request online HERE and someone will call you; or you can speak directly with a scheduling representative by calling 843-663-8000 and press 1.
Please keep in mind that first appointment availability varies depending on the provider you have requested. Telehealth is not an option for new patients.
TIP: Need transportation? We want to do everything we can to eliminate barriers to care. We know that having a ride to appointments can be difficult for some. Little River Medical Center is here for you. If you need transportation to any of your LRMC appointments, please let a scheduling representative know as you are requesting an appointment. Our friendly transporters have specific and limited schedules, so we will need to get you on the transport schedule!
4) Complete and review the new patient packet
All medical patients of LRMC must complete this form packet in its entirety. Please select and download the Patient Form packet and bring the completed forms to your next appointment. You may download and print the forms by clicking the link below. You may also arrive at your first appointment 30 minutes early to complete the new patient forms.
6) Appointment Reminder
You will receive an email and phone call reminder for your first appointment. Please follow the prompt in the email or the phone call to confirm your appointment. You can complete the registration process through the online check-in. If you do not have access to a computer or the internet, you complete the process in the office prior to your appointment. Please remember to allow at least 20 minutes before your appointment time to complete this process.
What to bring to your first appointment
- Any insurance cards for current coverage
- Driver license or other valid photo ID
- A list of prescription medications including vitamins you are currently taking (include the name and dosage)
If you are applying for our sliding fee program, please bring the necessary documentation. Following are forms of acceptable documentation:
- Preferably 2-3 current consecutive pay stubs
- Bank statements showing automatic gross income deposits
- Supplemental Nutrition Assistance Program (SNAP) documents that provide family size and income
- Your most current federal tax returns. If self-employed, a business income tax return with the appropriate additional tax schedule worksheet.
- Statement from employer
- Documentation showing additional income from alimony or child support.
- Statement from Social Security Office
- Current Unemployment Benefits statement
