Adult Medical Care
Regular primary and preventive care are important to maintaining our health as we age.
Our professional and experienced medical team provides care for routine, urgent and chronic healthcare needs for all stages of adult life.
Our primary care providers offer a range of screenings to help identify disease risk early. Depending on your age and gender, these may include screenings for high blood pressure, diabetes, high cholesterol, breast and cervical cancer, prostate cancer, sexually transmitted infections, depression, and osteoporosis.
Adult medical services include:
- Annual wellness exams and preventative care
- Disease management (diabetes, COPD, high blood pressure, and others)
- Care for minor illnesses (fevers, coughs, urinary tract infections, cuts, sprains and others)
- HIV/AIDs care
- Hepatitis C treatment
- Reproductive health services (contraceptives, STI treatment, family planning and more)
- On-site lab services
- Digital mammography
- X-ray services
- Bone density exams

- Medicare Annual Visit
Medicare Annual Wellness Visit
If you are a new Medicare enrollee, you are entitled to a “Welcome to Medicare” visit to meet with your health care provider to discuss and update your personal care plan. There is no charge for this visit and the Part B deductible does not apply.
Some of the topics addressed during this visit:
- A review of your medical and family history
- Height, weight, and blood pressure measurements
- A review of necessary screenings, vaccinations or other possible tests needed
- A simple vision test
- A review of falls risks and safety
- Screenings for hearing impairments, dementia and depression
- Discussion of advanced directives
This visit must take place in the first 12 months you are enrolled in Medicare Part B.
As a Medicare beneficiary you receive this benefit each year and it is classified differently from your annual physical. Your subsequent wellness visit appointments will cover topics that your provider established as a baseline during your Welcome to Medicare visit.
Chronic Care Management
Another Medicare Part B benefit is Chronic Care Management (CCM). If you are living with 2 or more chronic diseases, CCM can help you manage your long-term health concerns.
What are some of the chronic conditions that CCM helps address?
- Diabetes
- High Blood Pressure
- Heart Disease
- COPD
- Congestive Heart Failure
- Depression
- Cancer
- Arthritis
We will provide you with direct access to a personal care manager that is available to you by phone or email. Your care manager will work with you between your appointments with your provider.
They can provide assistance with:
- Appointments
- Care Coordination
- Prescription Refills
- Assistance with community services or support resources
- Medication management
- Meeting your health goals
You may pay a nominal fee for CCM, and the Part B deductible and coinsurance apply. If you have supplemental insurance, or have both Medicare and Medicaid, you may find the monthly fee is covered.

Same Day / Next Day Appointments
If you are an established patient, we can provide you with a same-day/next-day appointment. If you find it necessary to cancel your appointment, please call us as early as possible so that we may offer your time to someone else. We can offer telehealth visits if transportation or other issues prevent you from keeping your appointment.
After Hours:
If you are sick outside of normal office hours, please call the after-hours number to contact the provider on call. They will provide you with additional instruction. If you are experiencing a health emergency, please call 911.
