You are the most important person on your health care team. Patient-centered is our way of saying that you are the focus of your health care.

What is a patient-centered medical home?
A medical home is a team approach to providing total health care. At Little River Medical Center, we take care of everything you need when it comes to your health. When we say “patient-centered,” we mean that we listen to what you need and want and work together to keep you healthy.

How we provide your care…
Your LRMC care team is a team of dedicated health professionals who work together to meet all of your health needs. Your LRMC care team is committed to:

• Getting to know you, your family and your health and personal challenges.

• Listening to your questions and feelings, while supporting you as an active participant in your care.

• Communicating health care plan options and including your wishes in that plan.

• Providing support, education, and resources to help you learn more about your health.

• Coordinate your care through additional services.

What you can do…

You are the most important member of your care team. We can best support your needs if you help us with the following:

• Talk to your LRMC team about questions or concerns you have.

• Share your past health care successes and challenges.

• Let your team know about other health care professionals who care for you and how you feel about the care you are getting from them.

• Follow the health care plan that you and your team have discussed and make sure you understand how to follow the plan.

• Let your team know if you are having trouble sticking to the care plan or have issues with taking your medications as prescribed.

• Tell your team if your care plan is not working so that you can work together to create a new one.
