PATIENT CENTERED CARE
A medical home is a central location that provides a coordinated team working together to provide your care. Your health care team will be led by your primary care provider and will include nurses, behavioral health services, pharmacy services, insurance enrollment assistance, and case management to help coordinate any support services you may need. Additional team members such as dentists or substance use counselors are also available if needed.
Person-centered care is an approach to providing primary care that is based on the belief that your input and experiences will help your overall health outcome.
