Chronic Care Management
Living with more than one ongoing health condition means a lot to keep track of between appointments. Little River Medical Center’s Chronic Care Management program gives Medicare patients across Horry County a dedicated care team that stays in touch by phone, helps coordinate care, and catches problems before they grow. If you have two or more chronic conditions, such as diabetes, heart disease, high blood pressure, COPD, or arthritis, you may qualify at no or low cost under Medicare Part B. What is chronic care management?
Chronic Care Management, or CCM, is a Medicare benefit for people living with two or more long-term conditions. A member of your care team checks in with you by phone each month, reviews your medications, helps you follow your care plan, and connects you with resources between office visits. It is covered under Medicare Part B, and many patients pay little or nothing.

Our chronic care management program provides support for patients who have been diagnosed with two or more of the chronic conditions, including but not limited to, the following:
- Asthma
- Heart Failure/Heart Disease
- A-Fib
- Cancer (Breast, Colorectal, Lung, Prostate)
- Chronic Kidney Disease
- Hepatitis (B or C)
- HIV/AIDS
- High Cholesterol
- Diabetes
- Coronary Artery Disease
- High Blood Pressure
- Osteoporosis
- Chronic Obstructive Pulmonary Disease
Nurses provide individualized counseling and health education to help you better manage your conditions and follow your doctor’s care plan. Our team of nurses is here to help you improve your health and better manage your chronic conditions so that frequent hospitalizations can be avoided. The program is a voluntary service provided to eligible LRMC Medicare patients.
Who qualifies
You may qualify if you have Medicare and two or more chronic conditions expected to last at least a year. Our team can confirm your eligibility and explain any cost before you enroll.

How much does chronic care management cost?

Do I have to come into the office?
No. The monthly check-ins happen by phone, which is the point of the program, to support you between your regular visits.
What conditions count?

Same Day / Next Day Appointments
If you are an established patient, we can provide you with a same-day/next-day appointment. If you find it necessary to cancel your appointment, please call us as early as possible so that we may offer your time to someone else. We can offer telehealth visits if transportation or other issues prevent you from keeping your appointment.
After Hours:
If you are sick outside of normal office hours, please call the after-hours number to contact the provider on call. They will provide you with additional instruction. If you are experiencing a health emergency, please call 911.
