Our chronic care management program provides support for patients who have been diagnosed with two or more of the following conditions:
- Asthma
- Heart Failure/Heart Disease
- A-Fib
- Cancer (Breast, Colorectal, Lung, Prostate)
- Chronic Kidney Disease
- Hepatitis (B or C)
- HIV/AIDS
- High Cholesterol
- Diabetes
- Coronary Artery Disease
- High Blood Pressure
- Osteoporosis
- Chronic Obstructive Pulmonary Disease
Stroke Nurses provide individualized counseling and health education to help you better manage your conditions and follow your doctor’s care plan. Participants will have 24/7 access to healthcare professionals who work closely with you to develop individual goals to encourage healthy lifestyles. Our team of nurses is here to help you improve your health and better manage your chronic conditions so that frequent hospitalizations can be avoided. The program is a voluntary service provided to eligible LRMC Medicare patients.
